Your health records at DAJA HEALTH are the property of the practice; however, as a patient, you are entitled to have a copy for your records or to send it to other healthcare providers.

If you want records to send to another physician or yourself, complete the Records Release Out form provided below. This completed form can be mailed, emailed, faxed, or brought to the office's front desk for processing. It may take up to 10 days for your request to be processed.

A fee is associated with providing copies of your records from our office. This fee is dependent on the number of pages and the number of copies requested.
